UOPD Annual Evaluations

UOPD annual evaluations are to be completed, signed, and submitted electronically through PERFORM to Human Resources by April 15, 2019. If you have problems getting a hard copy of your evaluation, please contact your principal. If problems persist, contact DTU.

Any UOPD employee who receives an overall “Unsatisfactory” evaluation should have received a “Letter of Potential Unsatisfactory” by March 1 and should have been placed on a Performance Improvement Plan. If you received an overall “Unsatisfactory” and these procedures were not followed, please contact your DTU staff member (396-4063).
